Booth size is 10’x10’ with three chairs in each booth. The rental fee is $550 per booth for local artists and $450 for traveling artists. The City of Fargo has its own health regulations and any artist not from the Fargo/Moorhead area is considered a traveling artist. The cost of temporary licensing is $100.

- Artist must provide CPR and Hepatitis B Vaccination certifications before they are allowed to perform any procedure. If you need assistance finding your Hepatitis B Vaccination records you can contact that state health department in which you received the vaccination. Here is a link to all state health departments: immunize.org/states. To get a copy of your records you will have to fill out an immunization record request form.
Print the 2018 Technician License Form and fill out. Once you have been accepted you will need to send this document to the following Fargo Health Department contact to complete your licensing.
