Booth Rental:
Each booth rental is 10’x10′. No more than two artists can operate out of a 10’x10′ booth at the same time. Each 10’x10’ booth is $550. *Early Bird Special* Register and pay before June 30 and receive $50 off each 10’x10’ booth. The event will provide each booth with two tables, table cloths, three folding chairs, one biohazard receptacle, one trash receptacle, one sharps container, one small sign, and an event t-shirt for each paying operator. Operators are responsible for water, paper towels, hand sanitizer, and any other materials needed beyond what the event will provide.

The form below is just an application to participate in the event. If your application is accepted, the Health Department will require the following:
- Proof of current CPR, blood-borne pathogens certificate, and proof of Hepatitis B vaccination (three-part series). Failure to provide all three of these items will prevent you from working at the event. - Download Here: Completed Body Art Application and payment of $50 for the temporary operator's license.
Please email or snail mail all records and the application (all at once) to the health department. The deadline to submit paperwork to the health department is September 7, 2023.
